It originates from old-fashioned Russian bread, specifically a kalach, baked with a handle (ruchka) so people could hold it without touching the food with dirty hands. 

• The Story Behind It: In the past, the crusty handle of the kalach was often discarded or given to the poor because it was considered unhygienic after being held. Only those in extreme poverty, having completely "reached the handle" because of hunger, would eat it.

• «Калачная» версия (самая популярная): На Руси калачи выпекали в форме замка с дужкой («ручкой»). Горожане ели калачи на улице, держа за эту ручку, а саму её из гигиенических соображений выбрасывали или отдавали нищим или собакам. Тот, кто был вынужден есть эту ручку, считался опустившимся до предела — «дошел до ручки».

City dwellers would eat them on the street while holding onto this handle; for hygiene reasons, the handle itself was discarded or given to beggars or dogs.
